TLEs are available for this and other objects on these pages. Space-Track is no longer tracking this object. Actually, it's an odd case; they had TLEs up to 1969, then nothing until they briefly provided TLEs in mid-2017, then stopped again.

Click here for a graph of inclination and perigee height from launch in 1964 to re-entry in December 2047. The catalogued initial inclination for 1964-040A (on 1964 July 17) was 39.20 degrees, with a perigee altitude of 102000 km. The match to the plot is quite good. Because of this, and because the elements for mid-2017 match the Space-Track TLEs for that era, I'm confident of this identification.
